The royal headwear is an item that you get in Volcana by following the main questline.
First, you must get the udjat eye from the first area, found on level 1-2 or 1-3. Somewhere in the level there will be a golden key. Take that key to the door surrounded by what looks like a wooden version of the eye and unlock the chest inside. (https://static.wikia.nocookie.net/spelunky/images/c/c0/BigCh...). It will always look like this.
On 1-4 when fighting quillback, however you choose to defeat him doesn't matter but you have to go the rightside area. This is Volcana, going left will take you to jungle which will void this quest line for you and you'll have to start over.
Now, technically optional, but will save you some headache - on 2-1, you'll want to find a cave somewhere in the level that contains a silver key. That silver key unlocks a locked door also found on 2-1 and inside is Van Horsing. Walk up to him and he'll give you a diamond and will help you with the next part of the quest. (sometimes there are two keys and two locked doors on 2-1, one will contain money or an item and the other will have Van Horsing, its just bad luck, but usually there is only one)
Now on 2-2, 2-3, OR 2-4, there will be a giant drill at the top of the level, always in the top layer. Stand on the purple brick that is sticking out and the game will prompt you to press a button if you have the udjat eye. The drill will fall down to the bottom of the level. Follow it down and on either the far left or far right side, there will be another entrance into a backlayer. This is Vlad's Castle. Work your way all the way to the top of Vald's Castle and once Vlad sees you, Van Horsing will take his shot, killing Vlad, and dropping both his cape and the royal headwear, granting you this achievement.
Technically, you do not need the udjat eye, you can theoretically bomb your way down to the bottom of the level on the same level as the drill without activating it.
You also do not need to save Van Horsing, you can fight Vlad on your own and really, you don't even need to kill him, just reach up and grab the crown that's right by where Vlad spawns and you will get the achievement.

You can also purchase the Hedjet from the Black Market and this achievement unlocks.

You will get this trophy by picking up either the Hedjet, the Crown, True Crown or the Eggplant Crown of which the latter two are much more difficult to get. To find out how to get the Hedjet or the Crown, see the relevant section of Pilgrim
