How to unlock the Casanova achievement in Spelunky - Definitive Guide
Obtaining this achievement is entirely conditional due to the random nature of the game, and giving tips for it is hard for the same reason. You need 10 damsels in one go, so it's preferrable to start from the beginning where it's easier to rescue them.
This will only be achievable on a fraction of your runs, unless you're good enough to get very far into the game every run, so it's something worth trying for every time you play. They give you one life back when you beat the game every time, and there doesn't seem to be a maximum life you can have, so it's always worth saving them.
(Check out the options, you can change the damsels to pugs or male strippers if you prefer! Pugs all day!)

I would recommend going for the male damsels over the female one at least, as a) their screams are better at guiding you towards them (in my opinion) and b) their screams are not as obnoxious to listen to for hours in a row.

It unlocked for me as soon as the 10th damsel went through an exit, rather than waiting for me to take the same exit. Helpful to know if you take a shortcut...

To rescue 10 damsels, you need to play at least 10 levels, meaning you have to be able to get about halfway through the Ice world without dying, probably further. There is no way to go back a level or rescue more than one damsel per level.
Things that make getting the damsel sooner possible:
- the extra levels: Worm, Black Market, Haunted Castle
Things that make getting the damsel in levels difficult:
- dark levels
- angry shopkeepers (note that shopkeepers in 1 or 2 future levels will guard the exit if you killed one before)
- kissing booth level, you need enough money to buy the damsel (unless you're brave enough to steal her)
Thing that make not dying easier:
- each damsel will give you one heart back
- get sticky bombs, either by killing a giant spider or in a shop, to allow for precision and ceiling bombing to free damsels
- getting the Ankh on the Black Market (50,000 gold) which means you can die once (you will respawn at the start of the level, and the level doesn't reset)

If you grab the Damsel in the kissing booth (without stealing her, of course), it gives you the option to buy him/her/it for a bit more.

Note that feeding a damsel to a giant worm counts as saving her, even if you don't get eaten yourself; she will be there at the tally screen to give you a kiss. This allows you to rescue a damsel before you've reached the end of the level, making it a bit easier, especially if you have another item you want to carry as well.

Note: this is a bug report, not a solution.
This achievement appears to be bugged - I just received it in the Mines, after rescuing only three damsels (and sacrificing one). I'm not sure what circumstances led to this. The only guess I can come up with is that the night before I had rescued seven damsels, died, then shut the 360 off using the front switch.

To rescue 10 damsel in one game you will want to begin in the Mines as the later levels make it harder to rescure them. You can only rescue 1 damsel per stage, so the soonest you will be able to obtain this is at stage 3-2 in the Ice Caves level. Sometimes damsels will spawn in ridiculous places, buried in the terrain or up high where they seem almost unreachable. Make sure you have bombs and ropes; paste for sticky bombs is always useful. The achievement will unlock when the 10th damsel walks through the exit.
