I earned this achievement on the game mode Oddball, most people will be focused on the player with the ball so you can get kills while the enemies are distracted. It is also worth noting that assist kills seem to count toward this as well so if your allies finish a kill for you it still counts toward your streak.

I got mine in Oddball, but I got lucky because the other team was rushing separately.

Not super hard got in on my very first match. Probably best to try going for this while the game is fresh and people are still learning how to play

This achievement isn't too hard, hence the high completion rate. However, I have a solution that makes this achievement a downright breeze.
First off, contrary to popular belief, I think Atlantis is a much easier map to achieve this on, not Stadium. Here is a step by step guide on how to do this.
1. First off, make sure you are playing in Team SWAT. The one shot headshot makes this much easier.
2. When you spawn in, you will see two cubes you can place portals on. Place your first portal on the BACK side of one of these cubes.
3. Head out of the wide hall at the front of your spawn towards the large circular room at the center of the map.
4. Directly across from you, you will see the hall in front of the enemy spawn. Above this is a wall you can place portals on. Place your second portal there.
5. Now run back to the first portal (the one in the cube) and fire away.
The beauty of this strategy is that the enemy never sees it coming. This allows you to mow down the enemy as they leave their base. Plus, you can see them approaching your base so shoot them down before they reach it. The only way you can die is if they flank around you through the halls to your left and right or shoot at the portal. However, you can always hear them coming as long as you are listening. And since you will always get the first shot, they probably won't ever kill you using your portal.
Using this strategy, I got the achievement in my second game on Atlantis (the first I had streaks of 14 and 13 ended)

Perfect strategy, I actually found this strat through experimentation and got a 34 - 0 game using it.

I recommend using the map Stadium and getting portal kills. Place 1 portal on the big area out front of your enemies base and another on the inside of your base. You can shoot everyone from behind, but watch the minimap because someone will slip through and get to you.

I just unlocked this by play FFA and when juggernaut gets picked all you need to do is get 15 kill assists on the juggernaut. Just stay back and get your assists.
