Is it yet confirmed that Chaos difficulty is unlocked after beating the game on Universe, or can any difficulty be used?

To that effect, can I play on Galaxy, up the difficulty just before the final boss, complete it, then unlock Chaos mode? Or can I just play on Galaxy the whole time and unlock the hardest mode?

You can play on whatever difficulty you want. It only matters what difficulty you beat the final boss on. You can even play on "earth" difficulty and only change it when you face the final boss.

I needed to beat the final boss on "universe" difficulty in order to unlock "chaos".

Edit: I beat the final boss on chaos with Raymond (116), Nina (116), Laeticia (115) and Marielle (116). Using only purchased equipment. I grinded a few levels with the aid of Laeticia's passive skill "determined princess" at max level, which greatly increases the amount of XP your party earns, and a few es'owa pieces that granted XP boosts.

+1, beat it on Universe and unlocked Chaos, made a game clear save, loaded the save right before the boss and beat them on Chaos for the achievement just as described.

For anyone wondering, the boss can easily be brute forced around the level 110-120 range, which is what my party was. Not even an optimal team as I used Laeticia, Nina, Midas, and Raymond. Focus damage on his tail. Good luck!
