They added Task Force Operations to almost every main story arc. I am told these TFO missions are not required to get these story achievements.
There are 8 total missions(episodes) in the Klingon War story arc, but this achievement pops after mission 6. Many of them are given to you by Admiral Jorel Quinn (Earth Spacedock) and Franklin Drake or Commander Burgess (Starfleet Intelligence) after you report the previous completed mission to them. Most missions involve traveling to a part of space, doing battle with enemy ships in the area, then transporting to a location where you'll do some exploring and battling enemy personnel. Be sure to loot crates found in each area and to equip your mission reward gear and promotion skills as you get them.

There is a major bug in the Xbox version on the mission temporal ambassador in which you can't complete the last objective "scan for anomaly". Tried multiple times with no luck so if you happen to unlock this can you explain what exactly did you do in that mission?

Have you tried to sign out and back in again? I've found that quitting and restarting fixes some of this game's glitches. I've been stuck, had no prompts appear, mission stuff not progressing. Restarted and was able to continue.
