It would appear that this has been discontinued for the time being as the missions have been removed.

This trophy is called "Spectres" and it requires you to complete the now unlisted episode (story arc) titled Spectres.
As was kindly noted by KTKarona in the comments, during season 15, Age of Discovery, a lot of the game got revamped. Several missions were removed and others shuffled around to help make a more cohesive story with later released content. One of the moved items was the Spectres episode. It used to occur during the initial Klingon War story arc for Federation players, and later for Klingon & Romulan players, but is now pushed all the way back to the Klingon Civil War episode which is essentially end game story content after reaching Level 65.
Please note that I've read there is a bug where it may only unlock for Klingon players. I unlocked this as a Federation player a few years ago and am unable to personally confirm this.

Apparently the Accolade has been adjusted with the Age of Discovery title update. Now, to get the accolade and thus the trophy, it has to be unlocked by a Klingon/Romulus character, and to even get the questline started you must have them at end-game level 65.

This story arc is obtained by any faction at level 65. This story arc does not show up on the Episodes list. To obtain it, you have to go back to Admiral Quinn on Earth Spacedock for Federation and Jimpok in the First City for Klingons.

As of 5/14/21, there is a glitch where this achievement can only be earned by a Klingon faction character. I completed it first as my Federation and I did not earn it. I did it as my Klingon and got it as soon as I finished the last mission.
