This is 1 of the 4 Achievements (Empire, Warzone, Fek'lhri Return, and Vigilance) that must be completed with a Klingon character.
The Warzone story arc is the second story arc to be completed as a Klingon character. It is made of five Episodes (primary missions):
1. Bringing Down the House
2. The House Always Wins
3. Test of Mettle Edit: This episode is for the PC version only
4. Second Star to the Right, Straight on 'til Morning
5. Keep Your Enemies Closer
After completing the fifth mission, this Achievement will unlock and you will start the Spectres story arc (which can be completed by any character). You will need to complete the Spectres story arc before starting the third Klingon-only story arc, Fek'lhri Return.

I'm only seeing 4 missions in this arc. Test of Mettle is not in the Episodes list of this arc when you look in the in-game Mission log. Also, I just finished The House Always Wins and it went straight to Second Star to the Right, afterward.

This trophy is called "Warzone" and it requires you to complete the two missions under the Warzone Episode.
There used to be five missions, but with Agents of Yesterday's release, the final three missions were removed. If you completed the first two previously, but not the remaining three, the trophy will automatically unlock when logging into your Klingon account.
Now all that are needed are the following two missions:

To unlock this Trophy you need to Complete the Warzone story arc (as noted in the description) which is comprised of the following 5 missions...
