How to unlock the I'm a Living Legend achievement in STAR WARS Jedi: Survivor - Definitive Guide
There are 13 Legendary Adversaries in Star Wars Jedi: Survivor. I recommend going for this after completing the main story as some don't spawn until after the story is completed and you will need all of Cal's & BD-1's abilities to get to most of them.
2 on planet Coruscant, 2 on planet Jedha, The rest or on Koboh.
1) D-L1T (Coruscant - Hangar 2046-C)
2) Frenzied Jotaz (Coruscant - Undercity Meats)
3) Golden Skriton (Jedha - Desert Ridge)
4) Sutaban Alpha (Jedha - Crypt of Uhrma) This is the most confusing one to get to. You probably won't have the Crypt of Uhrma fast travel point if you still need this one. Closest meditation point would be Timeworn Bridge. Path is shown in my video.
5) Urgost (Koboh) You kill him during the main story. Unmissable.
6) Rancor (Koboh - Southern Reach)
7) The Mire Terror (Koboh - Viscid Bog)
8) Gorocco Matriach (Koboh - Derelict Dam)
9) Vile Bilemaw (Koboh - Fogged Expanse)
10) Spawn of Oggdo (Koboh - Fort Kah'lin)
11) E3-VE3 (Koboh - Untamed Downs)
12) The Massiff (Koboh - LucreHulk Core)
13) Beetu Deetu (Koboh - Southern Reach)
I found another Rancor in Phon'qi Caverns. So I had two Rancor kills before killing my last legendary enemy. Don't think you need to kill both (there might be even more) but if someone has unlocked this with one rancor kill and can comment that would clarify things.
Edit: Thanks to MIKE1989TW and a few others who are pointing out that you only need to rancor in Souther Reach, the Rancor in Phon'qi caverns doesnt count you need the one in southern reach.

I’d already killed the Phon’qi Caverns Rancor so left yours until last, I needed the one listed here to get it to pop and given the % increase each kill in tracking I believe the other one is not needed

You can keep track of who you've killed as each legendary enemy has their own page in the tactical guide which you can see after you've killed them. No need to scan with BD, it's done automatically.

I've seen a lot of comments (here, YouTube, other forums) about people not unlocking this achievement. I also had some troubles. Leaving three tips/pointers that will hopefully help.

Not all the Legendary enemies appear in the Tactical Guide.
This is partly what makes tracking this achievement confusing. Let me list what was in my Tactical Guide and what was not. My list matches what I could see reported elsewhere (e.g. https://forum.psnprofiles.com/topic/144026-im-a-living-legen...). Given this, if you do not have all the 11x entries listed below, then figure out which is missing and I'd recommend trying there first.
This list is in order:

There are 2x Rancors.
There is one in [Koboh > Sodden Grotto] and another in [Koboh > Phon'Qi Caverns]. There is mixed info about whether both are required. For me, defeating the second Rancor didn't increase the achievment tracker. That said, if it hasn't popped, I'd recommend ensuring you have both Rancors defeated. I think their bodies remain after you defeat them (although, not sure if this is time limited..).
For me, Sutaban Alpha behaved weird and was my 'glitched' enemy.
I definitely defeated this monster on my first visit to this area; I recall fighting it and also had collected the item in the arena. I visited the area again as part of my sweep for the achievement but the monster didn't spawn (expected). However, on my third visit, the boss did spawn and defeating it added it to my Tactical Guide. I had to leave the planet and return to get it to spawn.

The achievement is counted for killing all legendary bosses in one playthrough; if you killed someone during the first playthrough, you will need to kill them again in the new game+
