The easiest way to do this is to look at the Starmap and see where the green grid of unexplored section of a planet is. Then simply fly around the planet to make it 100% explored. Every time you find a new section of a planet, it shows at the top how much you have explored. It took me about 10 minutes of flying to do this to the first planet.

Could be buggy, as I could see whole planet, but achievement popped up only after I upgraded the last observation post.
You can do this manually by flying around until you've exposed every hidden area of the map on a single planet, or build and upgrade Observatories to uncover the map automatically. Three Observatories will cover a whole planet when fully upgraded. You can do this as early as Kirite or Haven, though, before even getting the ability to build outposts.
