Set your difficulty to Lethal(Obviously)
Now, hunt down or be hunted by:
Bloater
Screamer
Feral
Juggernaut
Screamers can be found in infestations
Juggernauts and Ferals can likely be found when attaching a Plague Heart(Lower it's health)
Bloaters can be found in a horde group or just in the wild.
Otherwise if none of these are working for you, get a car, and drive to numerous survey points as you will likely spot the freaks you will need.
Now, hunt down or be hunted by:

Not a situation I'd usually complain about, but I can't find a Juggernaut anywhere. I've killed several plague hearts and had a bit of a drive around, killed several of all the other types, but now I'm running out of survivors and supplies and still haven't seen a single one. I remember them being all over the place in Nightmare (normally right around the corner from my base). Am I missing something?
EDIT: I did eventually find my juggernaut just roaming around (and killed it with guns/explosives which worked fine for the cheevo). Just my observations from flailing around for a few hours, but I didn't see any Ferals until Day 3 (not even after killing a plague heart, which was a guaranteed spawn from day 3 onwards) and didn't see a Juggernaut until Day 7. I haven't been able to confirm these anywhere - was this just me being unlucky or is this a thing? (Sorry if this is a well known fact, I've normally been happy to avoid these guys so it's not something I've noticed before!).

A couple months ago, I started a lethal, and killed 3 of the 4 freaks. A screamer, feral, and bloater, therefore, I have 75% on the achievement. All I have left is a juggernaut. Yesterday I returned to the lethal, and killed 2 juggernauts for the achievement, nothing happened. I deleted the community. I then started a new lethal, and proceeded to kill a feral, a screamer, a bloater by melee, and a juggernaut on that file. Still hasn’t popped. I killed these juggernauts with different methods too. One was by car and explosion, bringing him to his knees where I piggy backed on him. The next was with guns, where I didn’t piggy back on him. And the last was melee, where I also piggy backed on him. After all that, can anybody help me out?

You must kill a screamer, bloater, feral, and juggernaut in Lethal difficulty.
This is reportedly a host-only achievement, like many other SOD2 achievements are.
Blood screamers can be found everywhere, and there are many ways to kill them - gun, crossbow, melee, car, explosive. Just be careful when attempting melee because they vomit blood plague when screaming, and you don't want to get that on you.
Blood bloaters can also be found everywhere. You really don't want to kill them via melee or car, but should stick with guns, crossbows, or molotovs. (Let's face it, though, you're going to run over one every so often.)
Blood ferals are not as common, but they will show up out of nowhere. Rapid-firing guns, especially shotguns, are a good way to kill them. Melee will work, but it's pretty risky. You can also run them over. They can and do dodge vehicles, but run up on them enough times, and you will get one. If one sticks to your car, brush up against a solid object (building, tree, guardrail, etc.) for an easy kill.
Blood juggernauts seem to be somewhat uncommon in lethal mode, at least in the early game. There are a few occasions when you're guaranteed to encounter one, such as the third mission with the mechanics enclave, when they ask you to help them power up their generator. The two-minute basesiege assault is another guaranteed blood juggernaut encounter.
When you see one, come prepared with a .50-caliber rifle. If you're having trouble finding one of those, the Daybreak prestige trader has them for sale. Hit the blood juggernaut with three headshots from the .50-caliber to get him to take a knee, then do the horsey-ride finishing move. I mention that finisher because I believe that is the only way the kill will count for the achievement. I killed two juggernauts without using that move, and they didn't count. (If that proves to be wrong, post a comment, and I'll correct.)

I’m not sure advocating shotgun use on Lethal is a good idea. Shotguns will spawn zombies (including more blood ferals) right on top of you. If you have a Red Talon contractor maxed out then maybe but overall it seems like a bad idea. Use 7.62 ammo with a Supressor, Gunslinger skill is best as you can tap A to aim automatically at the head (costs stamina, be careful).
I’d recommend the same for the Blood Juggernaut, if you can stand on something tall that you can jump off of for the final blow, .50 cal is fine, otherwise you’ll be killed long before he goes down.
The Blood Freaks on Lethal are not at all like the ones in Heartland, Heartland is a very low difficulty mode where noise pretty much doesn’t matter (except when downing a Plague Wall), in Lethal noise will get you super dead.
I’d recommend the same for the Blood Juggernaut, if you can stand on something tall that you can jump off of for the final blow, .50 cal is fine, otherwise you’ll be killed long before he goes down.

Actually, player weapon noise does not spawn zombies. It draws zombies that are already spawned, but it does not spawn new ones. New zombies are spawned by calls from alerted zombies, and when the zombie count is low, you can often kill any alerted zombies before they call their friends. You will encounter your first blood feral very early on, when there aren't many zombies in most areas, so gun noise isn't that big of a concern yet. Players won't be prepared for them upon the first encounter (Heartland, as you indicate, isn't comparable). The point of my solution is to discourage melee or vehicles with blood ferals, and also that with guns, rapid-fire is more important than hitting power. Again, because of lack of familiarity with how blood ferals move, getting a headshot the first time you meet one can be pretty tricky. A shotgun makes it easier. I definitely don't advocate for shotguns in lethal mode as a rule, but it's a pretty good way of killing your first blood feral.
