This one is fairly simple and straightforward, just start a new Lethal level game or take an existing community and upgrade all the difficulties level to Lethal.
For those curious, I earned this achievement by taking my existing community and updating everything to Lethal. When it prompts you the 'warning' that you will start a new community, accept yes and the game will load you into a new map. All your inventory items and any cars parked in a designated spot will transfer over. Shortly after the new map loaded the achievement popped for me.

Where in the menu or settings to you choose Lethal difficulty? I can't find it.

go into the community screen (where you can swap your characters) and the right most option (Tab) will be difficulty, next to your Base option. Change all three difficulty options (Community, Action, and Map) to lethal. It will prompt you with a warning that a new map will load. Accept the warning and you are all set.

Start the (main) campaign with the difficulty set to the highest, "lethal".
Just note that you only need to start up the session no other requirements.
