By far the easiest way to get this achievement is with the Heartland DLC.
In the Quincy & Helena storyline (The story arc I got the achievement in - maybe someone can confirm this also works in the Aunt Fi & Larisse storyline), you will need to recruit Isaac Robinson (by honoring his request to tell his husband that he's dead). That will give you the use of the plague annex in the medical tent. This will allow you to craft specific plague consumables for a cost of 15 plague samples and between 2-6 chemicals depending on the plague consumable you want to create. Therefore, you can pretty much craft as many as you want or need, enough to infect your character with the blood plague and pop the achievement.

I did this (in the end) the key really is to understand that contracting blood plague triggers when the count-down timer starts, I learned this the hard way by getting bitten and using the plague consumable immediately.
The way I did it, I simply went outside the base, ran into a few blood zombies and got attacked a few times until the blood meter was up to just under 3/4's of the way, then used the plague consumable. *TIP: It is easier to do if you don't have a follower with you as they tend to get in the way and kill the blood zombies you are trying to get to attack you. Hope this helps. (Used Malik in Heartland as he is super annoying with his constant comments so wouldn't have shed a tear if he did die
