Easy as pie. Simply select the multiplayer option at the start and connect. You don't even have to play. I tried to play multiplayer and the connection timed out. Still got the achievement.

Yes but first you must complete the game almost once.

You do not have to complete the game first. I haven't completed either Heartland or base game and just unlocked this.

Can confirm you don't need to complete the game first. I loaded up Heartland, and called for help. Achievement unlocked immediately when the other player connected and he hadn't even joined in.
