First make sure you are on at least Breakdown 5.
Stealth kills are when you crouch and hold LB and press Y when behind a zombie. You do not really need to be sneaky. Just be behind the zombie and crouched when you do the execution. You have three options and they are all easier than they sound. I thought this achievement would be very annoying but it's pretty easy as long as you stay near your base and don't run into large groups. Keep plenty of healing and stamina items with you to be safe.
1) Stay near your base and hunt down single zombies. When you get near a zombie, you can hold B to go into stealth mode and sneak behind the zombie without being seen and press LB + Y to do the stealth kill.
2) You can also run head on and press LB + A to kick them down then crouch by holding B and get behind the zombie and do the execution with LB + Y when the are standing up.
3) Let zombies attack fellow survivor and get behind them while crouched. You can press LB + Y to do the stealth kill while they are focused on survivor. Things are usually pretty crazy when other survivors are around and they can steal your kills.

in my experience stealth-killing hordes is the quickest way to unlock the ninja achievement. you can take out around 7-10 at a time. find a horde, either through driving around or using the map. get behind them, making sure zombies who aren't part of the horde aren't looking your way. always make sure you stealth-kill the zombie who is furthest back in the horde or they'll be alerted to your presence. work your way to the lead zombie and you'll even get a nice little animation for stealth-killing an entire horde.
