This is achieved at the end of the game, in the chapter JOHN: Let my Boy go!
Towards the end of the chapter, as City5 is begining to be destroyed, Richard will download his son back to the real world. After this, Sally will enter the dome, and Richard will be given a choice to download her or Lydia.
Choose to download Lydia and play through the rest of the game.
The achievement will pop when the apartment cut-scene is over and the credits begin rolling.

Near the end of the game, you’ll have to make some choices. For this trophy you’ll need to select “Download Lidya to Tracy”, then watch a few scenes until the trophy pops. The second choice doesn't matter for this trophy but there are trophies for each of them so choose the one that you didn't choose earlier.
