State of Mind

This takes place on Chapter 2. When in control of Richard you have to go to his work at The Voice. When you leave his apartment you will approach 2 tech dealers, one named Jace and enter an automatic cutscene. Once done with that continue to proceed to Richards work site.

Once inside head left and talk to his friend Steve (go make him a coffee too to proceed the story) about decoding Simon's memory chip. Go speak to him and get that started. While you wait "the hour to decode it" go talk to Richards boss in the large office at the back of the production floor. Richard will be in a lengthy cutscene and will be fired and you have to then go back to Steve and go through another cutscene unlocking more info.

Leave The Voice and once back outside go speak with a punk looking girl in front of 2 gates to speak with Jace the Tech Dealer you met earlier. They will proceed to betray Richard and knock Richard out thinking he is a cop.

You will enter a dream sequence as Adam in a tunnel where Richard and Tracy's cab crashed. Approach it and you will see a music box. Interact with it.

You will enter another dream sequence and be free falling as Adam. Look around for Tracy's music box and hit on it to grab it and end the dream sequence.

You will wake up as Adam in City5 and shortly thereafter the achievement will pop.

This achievement is story related so is technically not missable, but it is buggy and didn't pop for me the first time so it is missable, so if it doesn't work for you the first time, reload an older auto save so you don't have to replay the first chapter again.
