These are the red boxes that you discover in the lower levels of the mine. They are pretty easy to find and accumulate pretty quickly. Think it was my second map that unlocked the total.

I think the achievement is bugged, i'm about to finish my third play through and with each time I've fully opened up all of the mines, the tracker hasn't moved from 98%, it was at 98% at the start of my last playthrough too.
Edit: it seems restarting the game, loading up a save, and then clicking another chest seemed to make the achievement pop

@DistortedFive9 I'm also stuck at 98% and did teo whole campaign, the second one didn't count any. I exited the game and started a new game and opened another chest and it still won't register :/. This isnl my final missing achievements.
Edit: playing in sandbox mode disable achievements, start a new game at at least casual to get it.
