A gold star is given for the number of deaths equal to 0. Everything according to the description of the trophy, it is absolutely forbidden to die.

(!) If you suddenly get killed, crash, and/or some other misfortune happens to your character, quickly close the game until “Saving...” appears in the upper left corner. I also advise you to save your saves to a flash drive or cloud more often. Turning off the console, as one of the authors of the tip noted, is not necessary.

In caves with secrets and improvements, where there are abysses, if you fall into the abyss, a save occurs immediately and you appear at the beginning of the cave. I fell like this several times and thought that the trophy wouldn’t drop, but they gave it anyway, 0 deaths. falling into an abyss in such caves does not count as death. I played on easy difficulty.
