Just a heads up, this carries on to a new game. I was just missing fire and steam and it popped as soon as I reached the first steam death on my 2nd playthrough.

Should be mentioned that individual named creature deaths count as well. I had died to most of the monsters in the game (Trots, Rennick, OConnor) but somehow skipped Muir. Started a 3rd run, bee lined right through, and as soon as he killed me, ach popped.

I'll add that you also need to commit suicide (die) with the help of the poisonous gas that comes out of the pipes and blocks the way forward. Therefore, do not rush to close the valve when you first get to the Engineering Building at the beginning of the game.

Possibly patched. Got it a bit later than mid-game, so it's doubtful you need to die in the same ways you've died before.
