To unlock this you need to find Roy's Spoon which will take place pretty late in the game.
After you enter the area Accomodation for the third time you will be getting chased by a monster through a vent. Shortly after this you will drop down into the cabins where you need to find
to give to him for the main story. In the same room on a green chair will be his spoon which you need to just look at for a few seconds to get the achievement.

This is located close to the end of the game upon visiting accommodation for the 3rd time. Once you are in the room with Roy's medicine, turn around to see a green chair and you'll notice a spoon. Stare at this for a few seconds and your achievement will pop.

Damn missed this. Why the hell would they make an achievement for looking at a spoon when the rig hasn't actually sunk yet...
It would have made more sense for the achievement to be connected to picking up the insulin in his room...

During the escape from Trots towards the latter part of the game, after you've dropped out of the ventilation shaft and into the cabins, look for Roy's spoon on a tartan armchair facing a desk, by the bed. Once you've spotted it, this should pop.
