How to unlock the The Blessings of the Summoned achievement in STRANGER OF PARADISE FINAL FANTASY ORIGIN PS4 & PS5 - Definitive Guide
This is pretty easy to get, but can be a bit of a grind.
If you prefer EXTRA mode you can but note the items drop far less that you need to Identify the weapons and armour.
The smithy in the Lab run by the Tonberry has the new feature IDENTIFY which yes you guessed it identifies the items you have in that menu by using Acr powder, exlier's and crystals from rift enemy and chest drops.
Each weapon or armour has at the bottom 1 of 12 blessings, ( examples IFIRT, SHIVA, ODIN etc.) you need to identify one of each of the 12 different type of blessings.
You can just grind rifts before rift 21 on extra mode getting the required Arcanum while at the same time picking up more different gear to identify.
Do not quote me on this but i did not see the Phoenix or Bahamut blessings till i was on higher floors i think 18, but it can be random not sure, best is to get to floor 21 and grind this out.

The description for the achievement is wrong. You need to find and identify 12 itens during your runs around the Gilgamesh Difficulty.
The trophy description is a bit misleading here. You don't need one weapon with 12 different summons, but 12 weapons each with a different summon.
You can find blessed equipments during your journey, but they're pretty rare.
The best way to get them, is by identify unidentified weapons with the help of the Tonberry Blacksmith, in exchange for arcanium items.
Once identified, these weapons are called relics and you will see that they're blessed by a summon from the classic Final Fantasy games (e.g. Alexander, Bahamut, Ifrit). Each blessing offers a different boost.
To get this trophy, simply identify 12 different weapons with 12 different blessings. You can see them on the preview when select the unidentified weapon:

When completing the rift labyrinth, rarity blue artifact items will appear as loot, and they will have the blessing of one of the guardians. These items must be identified by a Tonberry blacksmith. To receive the trophy, you must identify 12 items with the blessing of different guardians.
