While going through the floors of the rift labyrinth, some of the portals will include red text that says "Chaos Event Chance Up". Play this room and run around until you find the chaos event. For me, a single large enemy spawned and the achievement popped when it was killed.
Do this before floor 10 because even with extra mode, it gets tougher.

Anytime you are inside a rift, there is a chance to trigger chaos events.
Those events will simply spawns extra hard enemies around your objective (like Tonberry or Cactuar).
You don't need to complete or survive the event: this trophy pops as soon as the chaos event begins, so you can run away if you find yourself in troubles.
