You will have plenty of chances to earn this throughout the game. However, it may be a little ways into the story until you find chest or door that has a hardest level Hack.
You will know you have encountered the hardest variety of Hack because it will be 3 sequences of 4 buttons (so 12 total).

If you're exploring/doing sidequests, you will almost certainly unlock this before you will be required to for story reasons (in Chapter 6, the Scoops Troop mission). This refers to a three-stage hack with four button prompts per stage. You will earn this trophy if you open the chest in the back room of the hardware store while working towards Rats! or the one in Granny Perkins' basement during Homebodies.
