This is a story related achievement while exploring the Seething Wastelands.
In order to get this you must explore most of the Seething Wastelands looking for three of the Xhinnon General lizards. When you kill one of them you gain an eye that will fit into a lizard statue at one of three towers. You must kill all three generals and activate all three towers for this to unlock.

Is this bugged? I can't get it to pop even after activating all three mirrors
Edit: this and the the last 4 story achievements all bugged on me. I had to do another playthrough to get them to pop
