This achievement is story related and is very near the end of the game.
Once you have defeated all three bosses, one in each of the major areas of the game, you will end up back in The Convergence which has been thrown into chaos after defeating Ingla, the third story boss. There is only one way to go through this area due to what is happening. Once you are prompted about the Point of No Return, proceed through the barrier to the Spire and this will unlock as you enter.

The last 5 story achievements bugged for me. I did a full playthrough on the easiest settings in one sitting and they all then popped for me

UPDATE
I redownloaded the game on Dec. 14, 2023 (about 3 weeks after the original post), and entered the spire from my last save and it unlocked! Don't know if a patch fixed it or something else did, but got 100% in about 10 minutes (because I still had to beat the final boss for the other achievement that hadn't unlocked).

Just wanted to post a warning that this "unmissable" achievement did not unlock after 2 playthroughs. I sent a bug report to the devs a couple weeks ago, but haven't received a response.
