This is an exploration based achievement towards the end of the game taking place in the Seething Wastelands. You will come to the Seething Wastelands as part of the story and unless you fully explore every zone, you could end up missing this.
Start by heading to the southern most zone of Seething Wastelands, Hollow Chasm. This whole area is an illusion and to shatter it, you must make it to the very bottom. When you get to the area with a stone tablet, touch it to make it disappear, which leads to a hidden tunnel. Make it through here all the way to the Rebirth Shrine and during the following cutscene, this will unlock.
