How to unlock the Psycho achievement in Streets of Rage 2 - Definitive Guide

Before you start a new game change the difficulty to very easy, this achievement can be gained in the first level after only a few tries.

Choose Max. On the first level when you come across the first guy with a metal pipe, take it off him.

Now you can melee enemies with that pipe, but I find it's more useful to use the jump and kick attack because if you try to melee an enemy with the pipe and they get to close you will grab them and drop the weapon. You don't have to kill them with the weapon to get the achievement keep in mind.

The jump kick is also very useful for the knife weilding mad man that comes along seconds later.

You can gain this achievement before even defeating the final boss (spikey that throws the knives) before you move on to the next stage.

This took me 15 mins to get and that was by trial and error, now you know the strategy it should take even less.

Wait until the first guy pops out of the sewers on the first level and steal his pipe weapon but don't kill him. From there you should be able to smack the rest of the enemies and get the achievement all before meeting the first sub-boss.

Just make sure you do not lose the weapon or pick up another one. The kills must be with the same weapon in a row.

Start a new game on very easy difficulty. Near the beginning of the level there will be a breakable object which contains a knife, pick it up and save the game. You can now attack and kill the next ten opponents using the knife for the trophy. Save your game after each successful kill. If you drop the weapon reload and try again. As the game is on very easy difficulty the enemies are weak and will not attack you as quickly.

This can be done at any point in the game but be sure to save after every kill so if something goes wrong, you can quickly reload and try again.

This is easy enough on the easy difficulties. Max is a good character selection as his swings hit front and back.

The key is caution, especially on higher difficulties. Most of the time, it's best to let them come to you, unless they have a range attack that your weapon will not cover. A pipe or sword is best, since they have a long attack range and do a lot of damage (excluding Skate). If you're having a hard time, you can save every time you get a few kills and reload as necessary.
