How to unlock the Unseen, unknown achievement in Styx: Master of Shadows - Definitive Guide
I'll define 'alert' as best I can - The bar above an enemies head represents their alert-status. This bar is allowed to turn FULLY YELLOW (Suspicious) and FULLY ORANGE (where they begin a search) without voiding the Shadow insignia. If this turns FULLY RED then they are 100% ALERT - if that happens, you will need to reload a save/autosave and continue on from there.
Tips:
I would recommend you try for this achievement on your second playthrough as you will most likely have the majority of upgrades and also know where to go and what enemies are where, but that is entirely up to you.
Upgrade Styx's Stealth all the way FIRST, then do whichever upgrades you prefer afterwards. Personally I think upgrading equipment/cloning next would be the most useful.
SECOND PLAYTHROUGH: When you pause the game, the insignia's are displayed at the bottom right hand side of the screen! If you think you've alerted someone, check to see if it's still on ZERO!
Try to keep your amber up at max (or close to it) as far as possible through missions so you can use it when the going gets tough!
SAVE OFTEN!! Can't stress that enough, you are likely to fail sneaking past enemies multiple times, be sure to save your game after every section so you don't end up losing 10+ minutes of progress! (Guilty of this )
In a tough spot? Use a CLONE to distract your enemies! Lead them away from areas so you can sneak past! Any alerts created by a clone WILL NOT count towards the insignia!
Not going for the mercy insignia? Dispatch of any enemies in your way! Just make sure their bodies aren't found!
This can be achieved on any difficulty! Do it on easy if you wish!
Also, in the first mission when you learn to kill, hide bodies and fight with someone, the two people there (guy by table and the one who comes in to fight with you after opening the gate) can spot you and you can kill them if you want to. They don't count either.
Ill go ahead and answer my own question after learning all insignias and side missiona are supposedly needed in order to learn al the skills

Excuse my bad English, i'm from Austria.
To unlock this insignia you must complete every zone in each mission without raising any alerts. An alert is whenever any enemy has a red detection meter above their head. The single exception is the "tutorial" battle of your first duel (it's unavoidable) in Reminiscences. For reference, here are the detection levels:
- Yellow - The enemy is suspicious and will look around (but not deviate from their path).
- Orange - The enemy is very suspicious and will investigate (walk to) whatever alerted them.
- Red - You're busted.
You should not go for the Thief insignia in the same run as the Shadow insignia, unless of course you simply want more of challenge.
Make liberal use of your ability to save whenever you wish so that you can reload if you do happen to alert an enemy. If your clone is detected that does not count against you. Do not forget that you have invisibility for the tough spots!
After you have successfully completed a mission for the first time, during replay of that mission the status of all four insignias will appear in the Pause menu. This is extremely helpful when going for this specific insignia as you can know for sure that no alerts have been raised. For this reason it might be good to go for the Thief insignia on your first play-through, and then the other three insignias on your second play-through.
