Head to the Phi Robotics lab. If you're having trouble finding it, there is a map at Delta Base (near one of the games first objectives). It is the green label, the symbol is a circle with a vertical line through it.

Inside the Phi Robotics Lab, you will find a map next to a scannable Pengling fragment (You will need to have a Spy Pengling later).
At the bottom left of the map is a circled area. This area contains the antidote.

There are two ways to get to this area;

1. In my opinion, the easiest way is to take a Seatruck to the pool just to the right of the circled area on the map (You will need a Seatruck with at least depth upgrade Mk1 as the tunnels can go about 200m deep). The entrance to the tunnels can be found about 1000m south-west of the Drop Pod and is swarming with Eye Jellys. Proceed west into and then through the tunnels, when you surface, the Spy Pengling entrance is very close and next to a frozen river.

2. You can get there by land but have to deal with Snow Stalkers, the cold, and the risk of getting lost! I wouldn't recommend this route.

In either case, you might have to deal with Snow Stalkers near the Pengling entrance. You can fend them off with flares and several good knife stabs.

Get out your Spy Pengling and send it into the cave, there is a small winding ice bridge you need to cross. Carefully traverse the bridge and at the other side is an open supply crate with a bottle of antidote in it. Grab the antidote and retrieve your Pengling. You will find the antidote in it's storage.

Now head to the Frozen leviathan. You can get there through the large cave almost directly opposite the Pengling Entrance, hug the left wall of this cave and when you emerge out the other side you will be in a large valley with purple trees. North West of this valley you will find a structure that looks like a radio tower which is right next to a cave entrance leading to the Phi Excavation Site and Leviathan.

Follow the cave and once inside, take the elevator up to where the leviathan's head is. Next to it, is a small robot that will accept the antidote. Install the antidote and use the control panel next to it to activate the robot.

When the antidote has been successfully administered, you should have your achievement!

For those that want a VIDEO guide, here you go! This guy's guides for this game have been consistent and good. He's very detailed.

Might be considered spoilerish but if you don't want to putter around in the water looking for stuff this will show you visually the where and what you need to do. Hope it helps... These vids and my guidance behind the spoiler wall is exactly what I did to get this one done.
