This achievement is unmissable as it forms part of the main quest line. It's unlocked after going to Lex' tower to remove the head bombs, early in the game.

This trophy unlocks in Chapter 3, when the squad are asked to track down Poison Ivy – or technically just Ivy, these days. You have to get to the LexCorp tower, and in the cutscene Harley will uncover a plant-infested lab and surmise that Ivy is, in fact, not dead but reccoperating somewhere. This trophy will then unlock.
