To get a perfect run up you have to get around 95-100 on the initial power/speed bar. Then to get the perfect take off you need to hit LT as soon as you can to plant the pole and get the posture bar to the top.

Perfect run up is easy enough, and it is pretty hard not to get a perfect take off. Just make sure you release when you are at the top of your jump. You should get this first time. Can be unlocked in any single player game mode.
