To get the trophy, you need to let the horror indicator reach 100. Which is almost guaranteed death, because... the team will start a riot (you can try to suppress it if you have high Hearts/Iron). The easiest way to get it is in the Frostfound location (in the north), passing tests (after each test, horror increases by 20).
