Constant Companion is an underwater creature who appears when you have a high terror rating. As the name suggests, it will continue to pursue you even when you leave its territory.
To encounter this creature, you will need a Zubmarine and a terror rating of around 75. It’s presence will be hinted at in your log and by a scratching, squirming sound. Eventually, it should appear and allowing it to kill you will unlock the achievement.
If this is your first time seeing the Constant Companion, you'll also earn the What Lies Beneath achievement.
