How to unlock the Chaos in the Old Factory achievement in Sunset Overdrive - Definitive Guide
Well with each round you play in chaos you get to vote on the type of mini-game.
Each map with give you a % chaos, and some other item. like +8 trap ..
The % chaos is what you are aiming for or higher (what ever the achievement requires)
the % after all rounds (before vat defense) needs to total 150%.
e.g
Round 1 = 15%
Round 2 = 50%
Round 3 = 35%.
Round 4 = 25%
Round 5 = 35%
vat protect = 0% (always)
total will be 160% thus gaining you the achievement as long as you complete the vat round successfully.
Of course with the mini-game being voted on, you find that some groups will automatically pick a game they find easier or a lower percentage of chaos.
I've personally had 148% because the group i played with picked a 35% over a 50%.
Also the very first round is picked automatically, you will get chaos for it, but you wont know if you hit over the 150% till the the vat protection time. So again, each round you actually get to pick make sure you aim for the highest amount of chaos as possible.

Rather than create a new solution:
What you need for this, and the other Chaos achievements, is to finish the game mode with x% of Chaos. You earn Chaos% just from completing each mini game. The 'score' you achieve and the 'bonuses' do not impact this achievement; you could technically sit idle for the whole game mode.
Each completed round or mini game adds to the Chaos% and you get to vote for the next mini game. Always vote for the round with the highest Chaos%. That's all there is to it.
There is RNG as the Chaos% seems to be totally random. You could do the highest Chaos% rounds every time and still not meet the target. I found Old Factory the hardest as we had to do 5 or 6 attempts.
You find out your Chaos% at the end of the mini games. The achievement should pop when you finish defending the vats. However, my session had issues and it took some players 30 to 60+ mins for the achievment to pop.
As a final note, it is probably best to quit and restart Old Factory if you don't get a >40% Chaos in the first couple of rounds.

My group voted the higher chaos each time, and we ended with 141 chaos.

Doing Chaos Squad with 2 people.
Nobody is playing this game now, and I am too lazy to go find a group. Me and a friend decided to tackle all Chaos Squad as two people and we have been successful. You have to set your Chaos Squad to private, invite the second person and then search. After a few moments it will put just you two into a game.
With only two people you have to be at the top of your game. You both will need all of your guns at level 5, and your other amps as maxed as you can make them.
The Guns we used were:
Hairspray Bomb w/ Second Amendment
Acid Sprinkler w/ Winter is Coming
Taunt Bot
Pulse Mine
Rager (Rager is better than Plague Bomb because Rage stops them from attacking the Vat. Plague does not.)
Swarming Nanobots
Freeze Bomb
Shield Buddy
You can use whatever amps you feel like, I never noticed too big of a difference except with freeze in the Acid Sprinkler. That is a must and will save your life.
For the personal Overdrives I would use OD Killer, OD Rival, OD Less Damage, Health upgrade, Deployable Ammo, and Single Shot Ammo. Damage on your guns is not terribly important because things are going to die anyway, you need sustained survivability and enough ammo to get through.
Always vote for the highest Chaos Squad in each mission. Generally if you vote that way you will reach the Chaos Squad goal. No matter what avoid Scavenger Scramble. The points one is unwinnable with two people. You really want the game to give you more Vat Capacity but you don't really have a lot of control over that.
The Night Defense has 5 waves. The first 4 waves have set waves of enemies. Once you defeat all the enemies the wave ends. The 5th wave is times. There is still a set number of enemies but it does not end until the timer is up. Even if you kill all the OD you can still lose if you get killed by Wingers. Speaking of Wingers, ignore them. Wingers, Herkers, and any other special OD. They will not attack the vat, so they do not matter. They will attack walls and traps, but the only thing that matters is killing the normal OD. This does make things difficult in later waves when there are a ton of Wingers around ruining your ability to grind, but you have to adapt and endure. With two people you can't focus on killing wingers and OD, and you gotta kill OD.
For each Night Defense I would put an Acid Sprinkler, Taunt Bot, and Pulse Mine in front of the gate. Then I would use Hairspray Bomb to attack. In the later waves when there is just too much going on I would focus on whats around the vat using Rage on large groups of OD. Acid Sprinklers around the Vat is a life saver.
For Traps I generally only used Senior Freeze. I would put them around the vat or in the path of the OD if the OD always followed a set path. Once there are a ton of Herkers and Wingers the traps get destroyed too fast. I would just put down a machine gun turret to help.
Old Factory Specific
You get 5 rounds before the Night Defense. Each round is not a lot of chaos so it might take a couple of tries to get all the way to 150.
Avoid: Destroy the Blimp, Scavenger Scramble 1, and Trolly Ride. They are not doable with 2 people.
Urban Jungle Gym 1, Supply Drop, and Clean the Sewer Plant are really easy.
For Cooking with Nukes you want to use a lot of Deployables to take out the OD who run in from all directions. The real secret is using the Amps Grind the Lightning or Fi-Yah!. You can just grind in a circle around the point. The fire or lighting (I used fire) will wreck the OD.
For Hack Fizzco use the Shield right before you start hacking to protect yourself.
For Signal Jammers the OD come to you in waves. Set up your Deployables in the direction they are coming to murder them.
For Scab Construction use Acid Sprinklers on the tower. It's pretty tough to other weapons, but the Acid just melts it.
For the Night Defense.
This one is as straight forward as it gets. The OD will come from the left or right and try to get to the one vat. You have plenty of things to grind above so use a lot of Deployables in front of the gates. Use Rager on the crowds and just destroy everyone. This is the true test to see if you are ready to take on all the Chaos Squads because they only get more challenging from here.
