The 5 weapons are: Spread Gun, Rockets, Laser, Grenade Launcher and The Shell. They all appear in flying pod type things above you throughout the game, 1 weapon in each of course.
Rockets and Spread Gun apppear almost right at the beginning of Level 1, Rockets appear first. Then the grenade launcher as you are climbing the small hill and then the laser soon after.
The Shell is found in level 2 in 1 of the 2 pods that appear when you see the turret/tank to the right of the screen soon after starting the level.
Your achievement will pop as soon as you pick up the last weapon.

Ah. It should be mentioned default settings need to be used for achievements to unlock. Never found that in any of the guides, but the settings menu told me

There are only 5 weapons to begin with so all you have to do is stay alive long enough to pick them all up which is just to level 2. You can get the rocket and spread in the start of level 1, a grenade launcher soon after, a laser after defeating the helicopter, and finally a bomb/nuke type weapon right in the start of level 2.
