Early in the game after riding the elevator out of the dungeon you will need to go though a portal in a phone booth to the Music Lab to make a button return to it's proper place so you can continue the story.
Earn five stars in the Music Lab by replaying songs from levels you've beaten in the story mode and you can unlock the chest directly to the right of the computer that craves TAPES, which will reward you with a cartridge.
Go right from the tape-obsessed computer to find the Game Garage. Enter it and pick the cartridge you unlocked from the chest up off the floor (it should be on the left side of the screen) and put it in the game console on the right side of the TV to unlock Maniac.
