Enter at the title screen :

40 Lives (Starts you with 40 lives)
Left,Left,Right,Right,Up,Down,Down,Up,Down RB,RB,RB (Lock,Lock,Lock)



Now once you have 40 lives start the game on a clean save.
Use Easy Difficulty.

DO NOT use the Weapon Code or stage select code as this disables getting this achievement.

All power Ups have to be collected in one playthrough.

NB - If you miss a power up push start and then select Quit. Now push continue and you will start at the last save point while having collected all power ups to that point.

Here is a list of all Power ups per stage :

Stage 1: M, R, O, E, L(Hidden), S, R
Stage 2: L, M, E, R
Stage 3: M, B, L, S
Stage 4: E(Hidden), R, S, M, B
Stage 5: M, E, L, --- Upper Path: S+R(This is suggested) or down path: O, M
Stage 6: M, S, E, S(Hidden), R(Hidden), L
Stage 7: M, E, B, S, L, 1UP(Hidden), M, R, S, L
