Enter at the title screen :

40 Lives (Starts you with 40 lives)
Left,Left,Right,Right,Up,Down,Down,Up,Down RB,RB,RB (Lock,Lock,Lock)


Level Select (Choose your starting level)
X,RB,RB,Y,Up,Right,Down,Left,Up,Right,Down,Left


Enter during the Game in the Pause Menu (Push Start during Gameplay)

Weapon Select (Lets you choose any weapon)
Right,RB,Left,RB,Up,X,Up,X,Y


When prompted which level start on Level 5 , then choose Easy difficulty.

Method :

Run through the level as normal. You will get to a point where the level splits and you can go up or down. Jump UP and take the Upper Path to fight this missable mini Boss.
