Enter at the title screen :
40 Lives (Starts you with 40 lives)
Left,Left,Right,Right,Up,Down,Down,Up,Down RB,RB,RB (Lock,Lock,Lock)
Enter during the Game in the Pause Menu (Push Start during Gameplay)
Weapon Select (Lets you choose any weapon)
Right,RB,Left,RB,Up,X,Up,X,Y
Method :
Start a fresh save with 40 lives on easy difficulty. You will have to play through the whole game atleast 2 times and a couple stages to earn enough score to unlock the Achievement. I unlocked the score on Level 2 on my 3rd playthrough. I used the Weapon select code so know that this does not lock you out of the Achievement.
