Enter at the title screen :
40 Lives (Starts you with 40 lives)
Left,Left,Right,Right,Up,Down,Down,Up,Down RB,RB,RB (Lock,Lock,Lock)
Level Select (Choose your starting level)
X,RB,RB,Y,Up,Right,Down,Left,Up,Right,Down,Left
Enter during the Game in the Pause Menu (Push Start during Gameplay)
Weapon Select (Lets you choose any weapon)
Right,RB,Left,RB,Up,X,Up,X,Y
When prompted which level start on Level 7 , then choose Easy difficulty.
Method :
After defeating what seems like the final boss the elevator will start flying up in to the air as the Real final boss pulls it up. All you have to do is to stand on either far side of the elevator platform. When you stand on either side the ends get broken off as the elevator flies up squashing you and getting you the achievement.
