This achievement is not terribly hard to earn as long as you do a little exploring in all of your story quests. I'm not positive which one I earned it on, but I'm fairly certain it was "X. Monastery Mystery".
I would consider this one story-based and not missable as long as you complete the story campaign quests. If somehow you do manage to miss it, replay the mission and make a point to explore more of the map as you complete it; sometimes small groups of enemies can be hiding in areas you don't get close enough to.
