How to unlock the Not a real doctor achievement in Super Meat Boy Forever - Definitive Guide

To unlock Dr. Fetus, you need to find 31 pacifiers and then you'll have to beat the 4th Boss (Dr. Fetus) and unlock Chapter 5. There is only 30 pacifiers per run so you will have to start a New Game+ after collecting the 30 in your first run.

You will know that there is a pacifier in a level by looking at the world map: if there is one, a pacifier icon appears below the level's title, in grey if you didn't collect it yet, in colors if you did get it. If there is no icon, there is no pacifier (but a pacifier could be in the other version of the level -dark or light world).

To unlock you need:
- Defeat the boss Dr. Fetus in World 4: The Lab.
- Collect 31 pacifiers

In one playthrough, a maximum of 30 dummies are available, scattered in both light and dark worlds. To make it easier, collect dummies on NG+ only in light worlds and run NG+ all the time until you collect the required number of dummies.
