Pretty simple, just hit a ball over the wall in fair territory. Easier to do when using a power swing. After the pitcher starts pitching, hold down 
and let go when you want to swing. Batters with more power are more likely to hit a home run.

When you do a power swing, do you move the thumbstick at all? Would holding the X button alone be good enough to hit it out of the park?

I found that if you pull the left thumbstick down just before letting go of the X button for the power swing you swing slightly under the ball and it sails out for a homerun. Using this method I also got the 4 HRs in a row achievement as well.
