How to unlock the Perfecto achievement in Super Mega Baseball 2 - Definitive Guide
This game saves after every pitch. If you're struggling with this, you can quit to the dashboard BEFORE the play is over and then reload the game and resume where you left off. I did this 3 or 4 times and the achievement popped for me.

How to I quit to the dashboard? And reload the game when I quit I can’t resume the game?

To return to the Home screen, press the Xbox button to open the guide, and then select Home.
Verify that the big application tile for the game is highlighted, and then press the Menu button on your controller.
Select Quit.
Relaunch the game and return to the game mode you were playing. I was playing in season mode. You can then continue the game where you left off.

I was having all kinds of trouble with this (even getting as far as 2 outs away from it and giving up a hit). I created a custom season where I took my MAX 99 rating team and copied it twice (so 3 99 MAX teams) and one 0 Bad Rated team. I simulated to the middle of the season where the 0 Bad team was 0-99 and their best player was batting .162 obviously this team was demoralized. I played a game and the batters only hit the ball into the infield twice and the rest were strikeouts.
I like to keep all my pitches low and inside and every once in a while throw it up to keep them guessing.

Just wanted everyone to know that I earned this using a 99 rated team against a 0 rated team in an exhibition game. Super easy

Right. I was able to get this with their ratings at neutral. Custom teams and setup are definitely the way to go.

I created a custom team and edited all stats for every player to be zero. I then chose the standard Sirloins team to play as and set the innings to 9 for the requirement. For the first inning, you will just need to score 1 run, which should be easy, as all of the stats for the other team are at zero and they have virtually no fielding or speed skills, and this is all you need to win since you will not be giving up any hits/walks/runs. Once you have the necessary run, just bunt (B) to complete the innings more quickly.
Just to be safe, pitch the corners using a variation of fastball and slider/curveball, depending on the pitcher. The players will be easily 'fooled' on most pitches and most of your outs should come from strikeouts. It is very rare for them to 'anticipate' the pitch and make contact, but since they have little hitting power, these should turn into easy fly-balls or in-field grounders.
A perfect game is where the pitcher does not give up any hits or walks. If anyone on the opposing team reaches base, for any reason, the perfect game is invalid and you will need to restart. Just be careful when you are pitching and it should only take about 20 minutes per complete game.
