Create a custom team and edit all player's stats to 99.
Load up and exhibition game and use 2 controllers. Chose your custom team as the home team and select the Away team as the "JACKS". Startup the game and you will be pitching first so allow your other controller to score 4 runs. Once they are at 4 just strike out the rest of the batters. Now that you're hitting use your second controller to walk the first 3 batters to make the bases loaded. With your second controller just tap A to pitch and with your main controller power swing and hit the home run. It should only require around 65-70 power to get the home run.
If you don't get the home run and end up scoring, just make your second controller get another 4 run lead, and repeat the steps to get your team with bases loaded and try to hit the home run. I got this on my first try with my custom team.
I recommend using "D. Oh" as the JACKS pitcher. Every pitch I power swing off of him is a Homerun.

Two controller was definitely the best way to get this. Although it took me until third inning for two reasons: first, I misread the description about bases loaded so I was hitting solo dingers. But also because I chose to bean every batter with 102 mph fastballs to the head and kept laughing at the goofy results. But regardless: two controller is the right call.
