This achievement took some doing - around 30 innings for me. For this to unlock, you need to hit a homerun while having a hitter on 1st base, 2nd base and 3rd base - all the bases 'loaded'.
To do this, I played on ego level 1 (easy), and picked the 'big hitters' team against the all-round team. I then used the
button to hit the ball into the outfield - but only let your hitter run to 1st base.
Do this two more times, without getting out, and you will have each of the bases loaded - all you then need to is time your shot well enough to hit a home run, and the achievement is yours.
