You get a Champion medal for completing all 10 stages in a world, including the bonus and boss, without needing a continue (dying is fine, as long as you still have lives left).
Be aware that to get this - except for your very first play - you need to specifically "go for the champion medal" by pressing X on the stage select screen. You'll know you're on track if you get a popup asking "erase your clear records for this world?", and then another after each stage saying "trying for the champion medal".
Do that 4 times, and you have the achievement.

You can also try practicing levels in stage select, then starting from an earlier, easier world to build up lives.

Does this have to be done in sitting?
I did Level 1-3 got the champion medals, came back to the game an hour later, completed world 4 with no deaths or continues no champion medal?
So do I need to complete world 1-10 in one go?

KiddingPigeon, no - but if you're starting from the stage select screen, you need to press X instead of A to launch the course.
You'll know you've done it right if you get a popup saying "erase clear records in this world?"
Then after every stage (until you continue), there'll be a pop-up saying you're currently challenging for the champion medal
