This is rather simple. You will more than likely achieve this naturally due to the odd control scheme.
But, avoid hearts, let a dust bunny hit the floor. Or run into a skull.

As the description says, simply die to unlock this achievement. This will definitely happen naturally on either stage two or three as you progress. You die either from letting a dust bunny land on a paddle or from touching a skull. You'll almost assuredly accidentally touch a skull during stage three.
