You have to kill 100 enemies with melee attacks to unlock this achievement. Whether the kills are bare handed or with melee weapons does not matter, as both count equally.
Likely the easiest area to grind these up is the campaign mission 09FIGHTC because you are in a cage with a bunch of unarmed guys and a bunch of melee weapons. As is usually the case, you can only level select if you've completed the mission already.

Simply get 100 kills melee, easy ways to do this are completing the "BAREHANDS" and "GHOST" challenges or playing on endless and only using the melee.

You will get this while going for all the other achievements. Alternatively, you can load up the FUNFUNFUN challenge after you beat the story. Punch 100 enemies to death and the achievement is yours.

If guns aren’t your bag, you can always punch the Red Dudes to death (it usually takes 3 punches to kill one unless a challenge modifies that). Punch 100 Red Dudes to death and this trophy is yours. This one will most certainly come naturally, if it doesn’t then boot up the FUNFUNFUN challenge for some easy grinding.
