A bit harder than the other 3 creation achievements but still pretty easy. Using the terminal in the lobby, click the creation mode button to create a level. Once inside the level place down a lockable door anywhere in the room. the doors are found by tab menu>decor>architecture>doors
After placing the door place down a power supply and a triggering mechanism. (this can be a button, an anatomy detector with a nearby organ to trigger it, etc) Then turn on circuit mode. (check the controls menu in the settings for which button turn on the mode for you) connect the power supplies output to the triggers input. Then for me, since I was using a heart detector, I connected the filled output of the detector to the unlocked input on the door and the empty output of the detector to the locked input on the door. Then go into play mode - for me it was F9 - and I put a heart in the detector and went through the door.
